We will help and guide you and your family during each stage of your journey. Our Outreach Team can offer you practical information and emotional support using a holistic approach.

We can offer you:

• 1-2-1 support meetings
• Signpost you to other services
• Provide language support where required
• Offer a range of information on conditions
• Be a listening ear when you need it the most
• Engage with other health professionals / services involved with your care
• Ensure your immediate needs are assessed and met
